Policies
The following Policies are also posted at the office for all clients to read. Please take a moment to familiarize yourself with these Policies, made in accordance with the CMTO guidelines.
Cancellation Policy
Your appointment time is reserved especially for you. Please note, that a 24 hour cancellation notice is required for any service provided. Otherwise, you will be subjected to a $30 missed appointment fee. Payment for a missed or cancelled appointment cannot by paid through your Insurance Provider. If cancellation of any appointment is received prior to 24 hours notice, then you may reschedule at no charge. Unforeseen circumstances that are deemed out of your control, will be addressed and negotiated on an individual basis.
Collecting Personal Information
At the first visit, you will be asked to fill in a detailed Health history. This information is required from the massage therapist to provide a safe and effective treatment. After each treatment, a detailed documentation is filled out by the massage therapist. This documentation is necessary to identify changes and improvement to the treatments provided to the client over time.
Files containing your personal information are safely stored to safeguard this information.
Confidentiality
None of your personal information is disclosed, without your signed and written consent unless :-
1) you request release of information and specify to whom you would like this information to be released
2) unless it is required to do so by the Court of Law
3) during the inspection of our records by the CMTO as a part of their regulatory
Quality Assurance Program
As professionals, massage therapists must report serious misconduct, in cases where someone's life or safety is threatened, in physically or sexually abused situations as well as misconduct of other practitioners (RHPA member).
Record Retention & Destruction of Personal Information
As a client, you have the right to access information contained in your health record. Massage therapist is required to make and maintain client records to the provision of massage therapy services.
1) Maintain client records for up to 10 years from the date of last entry, or if the client is less than 18 years old, 10 years from the date the
client becomes 18 years of age. Massage therapists may retain personal possession of the records or make arrangements for a custodian
to assume this responsibility.
2) Ensure that the confidentiality of client information is maintained.
3) Release information contained in the client's health record with the client's consent.
In the event of closing, selling or relocation of Rejuvenate Massage Wellness, you will be notified by e-mail or phone, up to 30 days in advance.

Consenting to a Treatment
Before starting a treatment, the Massage Therapist will explain to you :-
1) the nature of the treatment
2) the expected benefits of the treatment
3) the material risks and side effects of the treatment proposed as will as any alternative options
4) the likely consequences of not having the treatment
Please feel free to ask questions if you are unsure or need further clarification regarding the proposed treatment. I will be more than happy to answer any questions.
Once you have clearly understood the proposed treatment, then you can make an informed choice as to whether or not to consent to the treatment. This consent can be withdrawn at any time, should you wish to stop the treatment.
Sexual Misconduct
Sexual misconduct is forbidden. Client must understand that any illicit or sexually aggressive remarks, advances or gestures will result in the immediate termination of the session and will be liable for full payment of the scheduled appointment.
Payment & Receipts
Payment is processed upon arrival. Cash and checks are accepted at this time. Please make cheques payable to Tracy Muscat RMT.
After each treatment, you will be given a receipt, specifying that the treatment you received was Massage Therapy Treatment, as well as the duration and the price. The information on the receipt can be used to make an insurance claim.
For products and services that are not related to Massage Therapy (like products, oils, or Holistic Services are used by themselves and not complimentary to a massage), then the receipt issued will not read “Massage therapy treatment”, but will indicate the product or service provided.
When purchasing a Gift Certificate for someone special, the receipt will read “Massage Therapy Treatment for (whoever the gift is for) “. The receipt will be issued in the name of the person paying. With a purchase of a Gift Certificate, you will not be able to make an insurance claim.
For missed or cancelled appointments, without prior 24 hours notification, payment must be done by the client, and cannot by paid through your Insurance Provider.
The above policy on receipts are issued according to the guidelines of the College of Massage Therapists of Ontario and are given only in the name of the person receiving the treatment for the date of the actual treatment.
Punctuality
For subsequent appointments, please arrive a few minutes prior to your scheduled appointment.
All scheduled sessions begin and end on time. This is to ensure those waiting for an appointment can be accommodated in a timely manner. A late arrival will result in a shortened or rescheduling of your treatment. Regardless of the length of the treatment actually given, you will be responsible for the payment of a “full” session.
First Visit
On your first appointment, kindly arrive 15 min before your scheduled appointment. You will be asked to complete a brief medical Health History profile. An assessment will follow, which helps me have a better understanding of areas that require more focus. All this is done to ensure a safe and customized treatment tailored to your needs at the time.
Miscellaneous
Kindly refrain from smoking at least 2 hours prior to the treatment.
No services will be provided to clients who are under the influence of any substances such as alcohol or drugs.
Cell Phones and Pagers
To receive the full benefit of your relaxing experience, please set your cell phones to silent or vibrate mode.
Sickness Policy
Being human beings, both massage therapist and clients are vulnerable to infections. At Rejuvenate, we kindly ask the clients to call 24 hours ahead of time and reschedule their appointment when they are feeling unwell. In the event that the RMT is unwell, you will be notified in the same timely fashion, so that you can reschedule the appointment. This is to protect the health of both the client and the therapist.
Disclaimer
Always consult your physician, before considering massage therapy. Some conditions are not a good candidate or should warrant caution for massage. Massage therapy should not be considered as a substitute for medical examination or diagnosis.
